East Lancashire Hospitals NHS Trust seeking to recruit a Specialty Grade doctor within the directorate of Medicine for Older People. This is a full time post, with weekend and possible out of hours required.   

Medicine for Older People services are throughout all 5 sites of the Trust with the acute stroke unit and four acute MfOP wards on the Blackburn site and step down and rehabilitation wards on all other four sites.  The directorate is currently in the process of developing and strengthening its stroke pathway and frailty pathway, having achieved a level A for SSNAP and  will be opening a frailty unit early in 2019,  ensuring patients are provided with the correct standard of care in the right environment.  The directorate also provides ortho-geriatric support for the fractured neck of femur pathway.

This is an exciting time in Medicine for Older People. In recent years there has been a national push to ensure that the NHS is “Fit for Frailty” and, in the Northwest, East Lancashire Hospitals Trust is leading the charge.

We were selected as one of the first sites for the Royal College of Physicians “Future Hospitals” project and a lot of changes have been made to our service provision for older people as a result.

These changes include providing “front door” frailty assessment and review in the Emergency Department, enhancing community geriatrics support and care in our community hospitals and creating an Older Persons’ Rapid Assessment Unit to ensure safe, personal and effective care for the frail older people that present acutely. All of these align with our Trust’s Improvement Priorities.

You will be joining a dedicated and passionate multidisciplinary team who are committed to making our Frailty Pathway a reference point of excellence in the Northwest and indeed in the country.

Experience of working on busy Medical Wards is essential.  Full GMC registration with a licence to practise is required.The postholder will be  required to work on the middle grade tier within the Medicine For Older People wards, and provide the relevant expertise and experience necessary to ensure the efficient day-to-day clinical care of frail elderly patients throughout their pathway including within the Emergency Department and Acute Medical Units environment as well as within the community within the Intensive Home Support Service, working in partnership with other members of the multi-disciplinary team, and under Consultant supervision and guidance.

The Medicine for Older People directorate provides a comprehensive service that includes stroke, ortho-geriatric, geriatric medicine, frailty, dementia, delirium and falls. We are also a site for the Future Hospitals Project for the Frailty pathway. 

Royal Blackburn Hospital is a busy acute site with an active Stroke Unit with an out of hours specialist service that supports 24/7 thrombolysis.  The directorate prides itself on its dementia friendly environments and services for ortho-geriatric patients, geriatric patients and rehabilitative care. 

The department also plays a very active role in the local provision of Intermediate Care, with wards based on other hospital sites within the community. We work closely and cooperatively with the Primary Care providers and adult care services for out of hospital care.

You will provide support for the care of medical patients throughout their pathway within the directorate. There will be opportunities to gain experience in specialty outpatient clinics and working with our primary care colleagues.

The post holders will work in partnership with Consultant colleagues, matrons, ward managers, nursing and therapy teams as part of a multi-professional team.
