An exciting opportunity has arisen for two Band 6 therapists to join the Integrated Intensive Home Care Support Service (IHSS) based within the East Lancashire Locality.
IHSS is a dynamic, flexible, responsive and solution focussed integrated service, comprising of occupational therapists, mental health nurse’s, ANP’s (Advanced Nurse Practitioners), social workers and nurses.
The IHSS service is a rapid assessment model that works both within and across organisational boundaries, assessing and delivering patient care plans within the community setting, with the aim of avoiding unnecessary hospital admissions and expedite discharges from the acute hospital setting. The service works closely with Integrated Therapy Teams, Integrated Neighbourhood Teams (INT), Urgent Care and the Emergency Department (ED) at Royal Blackburn Hospital.
The working hours are 37.5 hours per week which may be worked over 7 days between the hours of 8am-8pm.
The vacancy is open to all band 5 therapists with experience in all clinical core areas/ those with the relevant transferrable clinical skills or those currently working at a band 6 level whom want to develop their clinical skills within the rapid response community setting. Duties will include ‘specialist’ level therapy assessments, delivering highly skilled evidence based treatment interventions and the management of adults within the community setting to avoid hospital admission or support discharges from the acute setting. The successful candidate will therefore be required to demonstrate clear ‘specialist level’ clinical reasoning skills.
Responsibilities would also include supporting and mentoring more junior members of staff and participating in wider service development.
Applicants must be able to demonstrate evidence of recent learning and examples of continued professional and clinical development. The IHSS service offers a comprehensive programme of supervision and clinical training opportunities and has a proactive approach to service development activities, in line with the trusts, divisional and service level objectives. Staff would be supported and encouraged to work closely together in order to deliver an integrated approach to patient care, following the six key elements of Care, Compassion, Courage, Competence, Communicating effectively and Commitment.
As we are a community based service it is essential that you are a car driver with access to a vehicle for work purposes.
