We are seeking exceptionally motivated band 5 nurses to join our team and support us to deliver the very best care to our acute elderly patients.
Our Medicine for Older People Directorate are excited to be developing an Older Peoples Rapid Assessment Unit (OPRA) which has 13 inpatient beds which are expected to be discharged within 48 hours from admission and 6 Rapid Assessment beds to support the recently established and developing frailty model. Your role in the team will be integral to implementing pathways for patients to deliver acute intervention, combined with an interest in care that supports rehabilitation to ensure Safe, Personal and Effective discharge and reduction in re-admissions.
You will be supported and welcomed into a fantastic team; working collaboratively with patients and their families, alongside the Multidisciplinary Team / Community Services. Education and development is key and you will be supported to develop your professional expertise through the varied and complex needs of our patients.
You will demonstrate effective communication skills and have the ability to develop relationships and communicate effectively with patients and carers, the team and the wider multi-disciplinary team to achieve the best patient outcomes.

As this position is exempted from the Rehabilitation of Offenders Act 1974 and involves work that is identified as a regulated activity as defined in the Safeguarding Vulnerable Groups Act (2006), any offer of employment is provisional upon a satisfactory DBS check being obtained.
The cost of an Enhanced Disclosure will be £48.00, which includes a small handling fee.
In line with other NHS organisations in the North West Region, the Trust is now passing the charge for undertaking a DBS check on to candidates in the event they are successfully appointed into the post for which they have applied. Candidates can choose whether to pay this over 1-3 months as a deduction from the monthly salary. You must have appropriate UK professional registration.
