Together A Healthier Future, the Integrated Health and Care Partnership for Pennine Lancashire, is committed to delivering the best health and care, with the best outcomes, staff satisfaction and resident experience. The Partnership is at an exciting point in its development as it moves to realise its ambition plans for health and care transformation, within the Lancashire and South Cumbria Integrated Care System. Our key priorities for transformation within the next twelve months include delivery of new ways of working for intermediate care, frailty, end of life, population health and prevention and providing the best offer for people within neighbourhoods.
We are one of seven areas in the country selected to work with NHS Improvement (Vital Signs) to design and deliver an innovative approach to transforming services and improving health outcomes in Pennine Lancashire. To lead on making our ambitious plans a reality, we are recruiting to a number of roles which will support the delivery of a new model of care for the people of Pennine Lancashire. The successful candidates will be some of the first people in the country to receive training and development through the national Vital Signs programme.
The post holders will be capable of working and influencing across the breadth of health and care organisations. They will work closely with the Integrated Care Partnership’ Portfolio Leads to support the delivery of a variety of transformation projects, being responsible for meeting key milestones, leading on or supporting the development of business cases, managing interdependencies with other workstreams and ensuring that plans, priorities and deliverables are achieved.
The successful candidates will be tenacious and comfortable working in a rapidly changing environment, supported leaders from across health and care who are committed to improving outcomes in Pennine Lancashire.
The following posts are for 37.5 hours per week and are open to recruitment on either a secondment or fixed term contract basis until 31 March 2020:
- 2 x Pennine Lancashire Transformation Programme Managers (Band 8A) – Closing date Friday 18th January
- 1 x Communications and Engagement Manager, Together A Healthier Future (Band 7) – Closing date Friday 18th January
- 1 x Project Support Officer, Together A Healthier Future (Band 5) – Closing date Friday 18th January
- 1 x Project Officer, Together A Healthier Future (Band 7) – Closing date Friday 25th January
